This is the photo and media consent form for a fictional youth club, Northside 12U Soccer — the first example. The fields are a parent or guardian and the children they name. You can type in it. It does not save or send anything. A field trip waiver sample is at /preview/field-trip.
A real form checks that the typed signature matches the parent's name, then stores the time. This page skips that on purpose.
